Reporting an Incident

Report an Academic Dishonesty Violation
Academic violations include cheating, plagiarism or falsifying official records related to the University, unauthorized signatures, tampering with course material, etc. General classroom misbehavior is considered a non-academic violation (see below). Report a Non-Academic Student Conduct Violation
Non-Academic violations include any behavior that is not related to an allegation of cheating or plagiarism (e.g. alcohol, drugs, threatening behavior, vandalism, disorderly conduct, etc.). Report a Concerning Behavior
The 51社区 Behavioral Intervention Team (BIT) has developed a form to be completed by any Faculty, Staff or Student who is concerned about an individual’s behavior. Concerning behaviors include, but are not limited to: emotional distress, suicide ideation, disordered eating, self-injury, threatening behavior towards others, and suspected substance abuse. Report a Unauthorized Animal on Campus
This form should be used to report concerns regarding possible unauthorized animals/dogs on campus or animals/dogs that are out of control and the handler does not take effective action to control it, or the animal/dog is not housebroken.
Please do not address the issue yourself. Service animals are not required to wear a vest or any identifying gear or tags.
